If an interpreter or participant support person must be present to support a participant during natural medicine services, a participant and facilitator must meet with the interpreter or participant support person prior to beginning the administration session pursuant to 4 CCR 755 (6.15).
During the preparation session, a participant and facilitator must work with the interpreter or participant support person to complete this form. Information provided by the participant, including but not limited to the Participant Information Form, will inform the participant's interpreter and participant support person needs.
What is a participant support person?
Participant support person means a person who will be present during a participant's administration session for any of the following purposes:
- Assist with administering medication to a participant
- Provide medical device assistance to a participant
- Assist participant with catheter, ostomy, or toileting assistance, ambulation, or transfer mobility support
- Assist participant with the participant's alternative communication device support or assistive listening device support during the administration session.
- Assist the participant to consume natural medicine products
- Assist in the observation of the participant for safety persons
Important Reminders:
- Interpreters and participant support persons shall perform only those activities identified in the written support person plan
- Interpreters and participant support persons are prohibited from touching participants except as required to perform activities identified in the written support person plan
- During the preparation session a facilitator should provide an interpreter and/or participant support person(s) with a copy of the participant Bill of Rights and answer any questions they may have
- A Participant who will participate in a group administration session must have the opportunity to meet other participants and any interpreters or participant support person(s) who will assist participants during the group administration session prior to the session commencing
- An additional form should be used if more than one interpreter or participant support person is required for assistance during an administration session
